Name Change Information

For information on how to changing your name with:
- Social Security visit the Social Security Online
- NJ DMV visit the New Jersey Department of Transportation, Motor Vehicle Services

Please also remember that other areas of your life besides your Social Security card, driver's license and DMV registration are effected by a name change, including but not limited to:
- Financial records, which includes: checking and savings accounts, loans and leases and don't forget about your credit cards accounts.
- Professional items, which includes: business cards and health insurance.
- Personal items, which includes: passport, personal memberships and phone book listings
